Ensure adherence tracking is recorded and monitored. Information is shared with
the patient and family, appropriately
Scheduled replenishments of all medication / tests to be undertaken by the
customer
Measure Key patient Parameters (vitals)
Remind / prompt scheduled lab tests or doctor visits
Identify first level trends in adherence and counsel the customer and initiate next
level intervention from the Company, if required, while sustaining adherence.
Provide information and explanation of diagnoses, options, outcome and risk and
escalate where required
Identify chronic disease patterns and suggest improved care
Talk about new products and services that are available beyond the current level of
services in the health care services.
Data updation in all tracking platforms on a daily basis
